موانع آموزش خودمراقبتی به بیماران دیابتی از دیدگاه پرستاران و بیماران

<p style="DIRECTION: rtl"> <strong> مقدمه و هدف </strong>: با وجود پیشرفت در تشخیص و درمان دیابت، کنترل متابولیکی ناکافی در مددجویان مبتلا به دیابت همچنان ادامه دارد. کنترل ضعیف وضعیت متابولیکی ممکن است نتیجه شکست در خودمدیریتی دیابت توسط بیمار و نیز استراتژی های مداخله ای ناکافی توسط کادر درمان باشد. یکی از دلایل عمده خودمدیریتی ناکافی در مبتلایان به دیابت وجود موانعی در آموزش به این مددجویان است. شناخت موانع آموزش خود مراقبتی به بیماران دیابتی از دیدگاه پرستاران و بیماران می تواند گامی مؤثر در ارتقاء آموزش خودمراقبتی به این گروه از بیماران باشد. لذا مطالعه حاضر با هدف تعیین موانع آموزش خود مراقبتی به بیماران دیابتی از دیدگاه پرستاران و بیماران انجام شد. </p><p> <strong> مواد و روشها: </strong>این پژوهش یک مطالعه توصیفی مقطعی است که در پنج بیمارستان وابسته به دانشگاه علوم پزشکی ایران انجام شد. 42 پرستار شاغل در بخش غدد و درمانگاه دیابت و 48 بیمار مبتلا به دیابت به شیوه نمونه گیری در دسترس انتخاب و پرسشنامه ها را تکمیل کردند. نتایج حاصله با استفاده از نرم افزار SPSS ویرایش 15 و با استفاده از آمار توصیفی تجزیه و تحلیل شد. <strong></strong></p><p> <strong> یافتهها: </strong>نتایج حاصل از این مطالعه نشان داد که از دیدگاه پرستاران موانع محیطی با میانگین 85/15و از دیدگاه بیماران موانع مدیریتی با میانگین 16/10، مهمترین موانع آموزش خودمراقبتی به بیماران دیابتی بودند. هم چنین نتایج نشان داد که دسترسی ناکافی پرستاران به منابع علمی به روز در زمینه دیابت ( کتب، مجلات و مقالات) در بخش غدد در بعد محیطی ، فقدان یک مرکز مشخص در بیمارستان برای دریافت آموزش ها، فقدان یک پرستار ثابت برای پاسخگویی به سؤالات بیماران دیابتی، پر مشغله بودن پرستاران و داشتن زمان ناکافی برای در اختیار قرار دادن اطلاعات در زمینه دیابت در بعد مدیریتی، مهمترین موانع آموزش خودمراقبتی از دیدگاه بیماران بودند.<strong> </strong></p><p> <strong> بحث و نتیجهگیری: </strong>بر اساس نتایج حاصله پیشنهاد می شود که مسئولان بهداشت و درمان و برنامه ریزان دانشکده های پرستاری با غنی کردن منابع علمی به روز در زمینه دیابت و برگزاری دوره های آموزشی برای پرستاران و تربیت پرستارانی متخصص در زمینه آموزش خودمراقبتی به بیماران دیابتی و قرار دادن آنها به صورت در گردش در بیمارستان جهت آموزش خودمراقبتی به بیماران دیابتی به کاهش موانع آموزش خودمراقبتی به بیماران دیابتی کمک کنند.<strong> </strong></p>
<p> <strong> Introduction </strong>: Despite the progress in the diagnosis and treatment of diabetes, metabolic control in diabetic patients is still insufficient. Poor metabolic control may be due to failure in diabetes self-care by patients and inadequate intervention strategies by health care providers. Barriers to self-care training contribute to inefficient self-management by diabetic patients. In fact, an understanding of these barriers can promote self-care in patients. Therefore, the current study aimed to determine barriers to self-care training in diabetic patients from the viewpoints of nurses and patients. </p><p> <strong> Methodology </strong>: This descriptive, cross-sectional study was conducted in 5 hospitals affiliated to Iran University of Medical Sciences in Tehran, Iran. Overall, 48 diabetic patients and 42 nurses working at the departments of endocrinology and diabetes clinics were selected via convenience sampling. The subjects completed the researcher-made questionnaires, and<strong> </strong>the collected data were analyzed by SPSS version 15 and descriptive statistical tests. </p><p> <strong> Results: </strong>The results showed that environmental (mean=15.85) and management (mean=10.16) barriers were the most important factors from the viewpoints of nurses and patients, respectively. Also, the findings showed that insufficient access of nurses to updated scientific sources related to diabetes (e.g., books, journals, and articles) at the department of endocrinology, lack of a diabetes training center at the hospital, nursing shortage (for answering patients’ questions), nurses’ work overload, and insufficient time for providing information were the most important barriers from the patients’ perspective. </p><p> <strong> Conclusion: </strong>As the results indicated, health care authorities and administrators at nursing faculties should update the available scientific sources, hold training courses for nurses, and specialize nurses in diabetes self-care in order to provide self-care training for diabetic patients and overcome barriers to diabetes self-care. </p>
